Group 5 Peer Evaluations

NOTE: THESE EVALUATIONS WILL NOT BE MADE PUBLIC. They are intended to let me know who did a lot of work, and who did very little.

Use the form below to evaluate your group members. If you are not a member of the group, DO NOT submit this form. YOU NEED ONLY SUBMIT THIS ONE FORM. Evaluate all of the other group members according to the questions asked. DO NOT evaluate yourself (If you do, it will just be ignored, so don't waste your time).

Evaluate each team member fairly (Don't evaluate them as great, when they weren't; don't valuate them as horrible when they weren't)

For the first group member (not you):

Last Name   Capitalize the 1st Letter of Group Member 1's Last Name

Now enter the First group member's evaluation in each of the following areas (numerically, according to the scale given)

1.  Attendance: Did the group member attend all scheduled meetings?
2.  Input:  How much did the member contribute to the ideas generated?
3.  Throughput: How much did the member contribute to the product design?
4.  Output: How much of the member’s ideas are reflected in the final product?
5.  Overall:  Regardless of individual area contribution, how much of the members work is evident in the product (e.g., through leadership, inspiration, etc.)

For the Second group member (not you):

First Name   Capitalize the 1st Letter of Group Member First Name

Now enter the Second group member's evaluation in each of the following areas (numerically, according to the scale given)

1.  Attendance: Did the group member attend all scheduled meetings?
2.  Input:  How much did the member contribute to the ideas generated?
3.  Throughput: How much did the member contribute to the product design?
4.  Output: How much of the member’s ideas are reflected in the final product?
5.  Overall:  Regardless of individual area contribution, how much of the members work is evident in the product (e.g., through leadership, inspiration, etc.)

IF you have a third member to evaluate in your group, you MUST answer the same questions about Group Member 3. If you do not have a third member to evaluate, you can submit the form now.
